Social Worker (LCSW) - Day shift/ Need to have someone who has Acute Hospital Care experience and be familiar with MediCal and Medicare.
Position OverviewThe LCSW will assess patients’ psychosocial needs, coordinate discharge planning, and provide crisis intervention and counseling services.
ResponsibilitiesConduct psychosocial assessments
Develop safe and effective discharge plans
Coordinate with case management, nursing, and providers
Provide counseling to patients and families
Facilitate referrals to community resources
Maintain accurate documentation
Active California LCSW license required
Acute care hospital experience preferred
Strong discharge planning experience
Knowledge of insurance and community resources
