Remarkable Hospice and Home Health seeks an RN Case Manager in Victoria, TX to administer hospice care across home and facility settings. The role coordinates with a multidisciplinary team and families, ensuring a patient‑centered Plan of Care is followed and documented. Independent work with on‑call rotation is expected.
