Physical Therapist - Travel Contract

Join a home health team in Burbank, CA as a traveling Physical Therapist where you will help patients recover from injuries and manage chronic conditions while providing in-home rehabilitation services. This role emphasizes independent clinical decision-making, documentation, and meeting productivity expectations in a community-based setting.

Minimum Qualifications
Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active California Physical Therapist license required to start the assignment.
Current BLS certification from AHA or ARC may be required.
Home health experience preferred.
HCHB (HomeCare HomeBase) experience required.
Experience with productivity expectations in home health, including ability to meet daily point targets (example productivity model: combination of follow-up and Start of Care visits).

Preferred Qualifications
Experience managing caseloads in community or home health settings.
Familiarity with Start of Care documentation and home health visit coordination.

Assignment Details
Schedule: Monday to Friday, 8:00 AM to 5:00 PM.
Shift pattern: 8-hour shifts, Monday through Friday.
On-call or weekend requirements: None.
Territory: Santa Clarita/Valencia coverage expected.
Productivity expectation: 6 points per day (may include combinations such as 5 follow-up visits, 2 Start of Care visits, or 1 Start of Care and 2 follow-ups).
