Key Responsibilities
- Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the RN and other members of the interdisciplinary group, wither verbally or through documentation in EMR. Provides an assessment in the patient's identified residence and provides assistance when the residence is not safe and another plan is required.
- Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risk for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provide crisis intervention when necessary. The goal is to provide education and support in order to prevent a crisis.
- Coordinates respite stays as per family/caregiver needs, facilitating the review of medical records and all that is included in the preparation of and follow-through of the respite stay.
- Provide bereavement counseling.
- Educates patients on, and assists in, preparation of advanced directives and durable power of attorney for healthcare, if not previously executed, and only if the patient is cognizant.
- Provides information and referral services to organizations to patient's/families/caregivers regarding practical and environmental needs, i.e., private duty agencies, Medicaid programs, Meals on Wheels, Elder Law, etc.
- Maintains clinical competency.
- Assists in coordination of care with personnel at nursing facilities or patients' other living settings.
- SW participates in the planning of the live discharge of the hospice patient.
- SW provides the emotional support of anticipatory and cumulative grief/loss depending on the wants/needs of the patient/family.
- Provide high-quality, compassionate care to patients in a facility or home-based setting.
- Collaborate with a multidisciplinary team to deliver holistic care.
- Support patients and families with personalized, one-on-one interactions.

Requirements
- BSW, MSW or LCSW required
- Hospice experience preferred
- Alignment with the values and mission of Catholic Community Health.
- Ability to work effectively in an environment that is very diverse
- Must possess a valid Driver's license, dependable transportation, current vehicle insurance, and a verified safe driving record.
- Strong organizational and time-management skills with the ability to multitask effectively.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively in a fast-paced environment.
