About the Role
We are seeking an experienced Physical Therapist for a 13-week Home Health contract in Contra Costa County, California . In this role, you will provide individualized therapy services in patients' homes, helping improve mobility, independence, and overall quality of life. The ideal candidate is organized, patient-focused, and experienced in managing a home health caseload while maintaining accurate clinical documentation.

About the Role
We are seeking an experienced Physical Therapist for a 13-week Home Health contract in Contra Costa County, California . In this role, you will provide individualized therapy services in patients' homes, helping improve mobility, independence, and overall quality of life. The ideal candidate is organized, patient-focused, and experienced in managing a home health caseload while maintaining accurate clinical documentation.
Responsibilities
- Perform comprehensive patient evaluations and develop individualized treatment plans to support functional recovery.
- Deliver skilled physical therapy services in the home health setting while prioritizing start-of-care visits.
- Supervise Physical Therapy Assistants (PTAs) and collaborate with the interdisciplinary care team.
- Complete clinical documentation, including start-of-care reports and treatment notes, within required timeframes.
- Maintain ongoing communication with clinical supervisors and office staff to ensure coordinated patient care.
- Active California Physical Therapist (PT) License required.
- Minimum 1 year of Home Health Physical Therapy experience required.
- Ability to travel throughout Contra Costa County and surrounding areas as needed.
- Strong documentation, communication, and patient care coordination skills.
- Ability to manage productivity expectations while delivering high-quality patient care.
