Clinical Liaison Responsibilities:

• Collaborate with healthcare professionals, residents, and families to coordinate comprehensive care plans that meet the unique needs of each individual

• Develop and maintain relationships with healthcare providers, hospitals, and other external agencies to ensure seamless transitions and continuity of care

• Conduct initial assessments, identify risks, and develop individualized care plans to mitigate potential health issues

• Educate residents, families, and staff on healthcare procedures, medication management, and wellness strategies

• Maintain accurate and detailed records of patient interactions, assessments, and care plans

• Participate in multidisciplinary care conferences and team meetings to ensure effective communication and collaboration

• Identify and report any concerns or issues related to resident health and well-being to the healthcare team and management

Clinical Liaison Requirement:

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Missouri.

• Experience and skills as a clinical/healthcare liaison in a long-term care/skilled nursing or related field.
