Contract (14 Weeks) | Full-Time | Days | Monday–Friday | Onsite

Key Responsibilities

• Provide physical therapy assistant services to home health patients across Santa Clara County
• Manage a caseload of approximately 25 patients, seeing 5–6 patients per day
• Travel to patient homes throughout the assigned service area; mileage reimbursed at the federal rate
• Document patient care and progress using HCHB (HomeCare HomeBase) software
• Collaborate with the supervising Physical Therapist and interdisciplinary team to support patient goals

Required Qualifications

• Active PTA license in the state of California
• Prior home health experience required
• HCHB (HomeCare HomeBase) experience required
• Valid driver's license and reliable transportation for home visits

Preferred Experience / Skills

• Experience managing a high-volume home health caseload
• Strong time management and independent clinical decision-making skills
