Revisit Nurse

Hartford Healthcare seeks a compassionate Revisit Nurse to provide follow-up care for patients across our Healthcare & Medical Services network. In this role, you will conduct scheduled revisit assessments in clinic, home, or virtual settings, monitoring symptoms, vital signs, medications, and wound care to support safe recovery and reduce readmissions. You will reinforce discharge instructions, educate patients and families on disease management, and coordinate with an interdisciplinary team to adjust care plans.
