Home Health Licensed Practical Nurse (LPN) - Matthews, NC

Job Description

Deliver compassionate in-home care as a Licensed Practical Nurse specializing in home health services. This contract assignment is an excellent fit for a skilled LPN with a passion for geriatric patient care and strong proficiency in electronic medical record systems, specifically HomeCare HomeBase (HCHB).

Schedule/Hours:

Contract assignment

Required Qualifications:

Current, active LPN license
At least one year of recent home health experience
Demonstrated proficiency with HomeCare HomeBase (HCHB) EMR
Solid clinical assessment and documentation abilities
Reliable transportation for in-home visits
Strong ability to work independently while effectively managing a patient caseload

Primary Responsibilities:

Deliver skilled nursing care in patients residences, adhering to established care plans
Monitor patient progress and communicate changes, updating plans of care as necessary
Administer approved treatments and medications per state regulations
Provide patient and family education in managing conditions and preventing disease
Accurately document care and patient status within the HCHB EMR system
Collaborate with RNs, therapists, and other healthcare professionals to coordinate comprehensive patient care
