Home Health Coordinator- RN

Sentara Home Care is seeking to hire a qualified individual to join our team as a Home Health Coordinator- RN.

Position Status: Full Time - Day Shift

Position Location: Williamsburg, VA

Mileage reimbursement will be paid for all miles driven between patient visits.

The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards. Includes interaction with other facility staff. Actively participates in interdisciplinary team meetings. Conducts initial and periodic assessments. Administers nursing care to ill, injured, convalescent or disabled participants. Advises participants on health maintenance and disease prevention. Administers medication if needed.

Education:

• Associate's Degree Nursing (Required)

Certification/Licensure:

• RN (Required) - Active in VA
• BLS (Required within 90 days)

Experience:

• 1-year minimum nursing experience (required)
• Home Health experience (highly preferred)
