Mileage reimbursement./ppResponsibilities include:/pulliProviding skilled nursing care and treatments to patients of all ages as assigned by the RN./liliAssisting the RN case manager in coordinating all aspects of patient care./liliCompleting and submitting accurate clinical documentation within agency timelines./liliComplying with all hospital, departmental, and regulatory policies and procedures./liliParticipating in continuing education, staff meetings, and required competencies./liliProviding customer service with respect, kindness, and empathy to patients, families, and staff./liliProviding care in patient homes, which may involve limited space, temperature extremes, environmental hazards, or equipment transport./liliDriving between patient homes daily, often in variable or adverse weather conditions./liliFollowing infection prevention practices and safely handling blood and body fluids./liliWorking independently with minimal direct supervision and collaborating effectively with the care team./li/ulpRequirements include:/pulliEducation: Graduate of an accredited practical/vocational nursing program./liliExperience: At least one year of medical-surgical nursing experience./liliLicensure/Certification: Current Licensed Practical Nurse (LPN) licensure in the Commonwealth of Virginia required. Must hold a valid Virginia drivers license./liliLife Support: AHA BLS-HCP required when providing direct patient care./liliOther Minimum Qualifications: Must demonstrate the ability to perform thorough assessments and technical nursing care under RN supervision. Must be able to work independently in home environments, manage documentation requirements, communicate effectively, and interact professionally with patients, families, and other staff.
