Physical Therapist w/ $5,000 Sign On Bonus

BoldAge PACE is an all‑inclusive program of care, personalized to meet the individual health and well‑being needs of our participants. Our approach is simple: we listen to participants and their caregivers to truly understand their needs and desires. The Physical Therapist will plan, organize, and implement physical therapy services for PACE participants. Responsibilities include assessment, treatment planning and delivery, and education of participants, families, and staff. Responsibilities Provides high quality care and serves as a member of the PACE interdisciplinary team (IDT). Performs in‑person assessments for participants at initial enrollment and as needed. Assesses need for adaptive durable medical equipment, specialized shoes, prosthesis, and home safety. Recommends home modifications and equipment. Plans physical therapy treatment programs according to recognized standards and policies. Administers treatment programs consistently to achieve identified goals. Confer with interdisciplinary team on participant care issues. Documents evaluation, assessment, plan and treatment rendered accurately and in a timely manner. Meets effectiveness of treatment, documents and revises plan accordingly. Educates and instructs participants/families in procedures, home programs, and obtains training and understanding. Trains PACE staff in body mechanics, equipment use, transfers, gait patterns, and methods to maximize safety and functional abilities. Provides care and services at the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care. Assists participants to function at most independent level possible. Develops and maintains positive relationships and communication with co‑workers, participants, families, and community members. Collaborates with the interdisciplinary team to develop a comprehensive care plan. Attends and actively participates in daily interdisciplinary team meetings, organizational meetings, in‑services, and community agency meetings. Completes all documentation of clinical service in participants’ medical records according to PACE documentation requirements. Follows all policies, procedures, OSHA safety guidelines, and protects privacy and confidentiality. Maintains a safe working environment and follows safety policies and procedures. Participates in continuing education and maintains professional affiliations, required certifications, and continuing education requirements. Performs other IDT duties as needed and other duties as assigned. Qualifications Bilingual and fluent in Hmong preferred. Advanced degree in Physical Therapy from an accredited school of Physical Therapy. Current license to practice Physical Therapy in the state. Current driver’s license and proof of auto insurance. Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community‑based setting preferable. One (1) year of experience working with a frail or elderly population preferable (training provided if not present).
