- Provide social work services to residents, including assessments, care planning, and interventions to meet their emotional, psychological, and social needs
- Collaborate with interdisciplinary teams to develop and implement individualized plans of care
- Conduct psychosocial assessments, identify resident strengths and needs, and develop strategies to address them
- Provide counseling, support, and education to residents and their families to help them cope with changes and challenges
- Facilitate communication and problem-solving between residents, families, and healthcare teams
- Maintain accurate and timely records of resident services and progress
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Licensed Social Worker (LSW) in the state of Texas or eligible for licensure
- At least 0 years of experience in social work, gerontology, or a related field
- Strong communication, interpersonal, and problem-solving skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks
