Physical Therapist

A Brief Overview Assesses patients and establishes appropriate treatment plan based upon diagnosis and professional standards. Provides appropriate treatment of patients using specific modalities and treatment techniques. Accurately documents treatments and care of patients and communicates with other team members. Participates in individual and department continuous improvement and education activities.

What You Will Do

• Direct patient care, including the assessment and treatment of patients, developing and carrying out individually designed treatment programs.
• Performs and documents an initial evaluation, interprets data to identify problems, and determines functional limitations prior to intervention.
• Performs ongoing documentation in the patient medical record with functional limitations, goals and appropriate timelines
• Communicate with referring Physician to obtain certification of plan of care and document as appropriate.
• Includes the patient/caregiver in treatment process, evaluation of effectiveness and discharge as necessary.
• Evaluates the effectiveness of treatment at various stages and makes adjustments to achieve maximum benefit or discharges when treatment no long needed
• Ensures patient education, proper fit and use for assistive devices as necessary.
• Supervises PTA, PSM Aide, students and volunteers as the clinical schedule dictates
• Participates in team and hospital wide meetings, committees and initiatives as assigned
• Assists with department finances with accurate daily billing practices, active daily maintenance of patient care schedule.

Education Qualifications

• Other Graduation from a physical therapy program from an accredited college or university Required

Experience Qualifications

• 3+ years to 5 years of experience Required

Required Knowledge, Skills and Abilities

• Criteria to attain level 2 requires 3 years experience, with one year at SHC TV preferred; and 1 of the following 7 criteria be met at application.

• The APTA's board of PT specialties recognizes 10 specialty certifications for Physical Therapists
• Gerontology (BCG)
• Mental Health (BCMH)
• Pediatrics (BCP)
• Physical Rehabilitation (BCPR)
• Driving and Community Mobility (SCDCM or SCDCM-A)
• Environmental Modification (SCEM or SCEM-A)
• Feeding, Eating, Swallowing (SCFES or SCFES-A)
• Low Vision (SCLV or SCLV-A)
• Other Certifications/Course series Included for Consideration are:
• o Lymphedema
• o Vestibular
• o Stroke
• o Developmental care specialist
• o Pelvic Health
• o Ergonomics
• o LSVT BIG/LOUD
• APTA Advanced Proficiency Pathway for PTAs
• American College of Sports Medicine (ACSM) Exercise Specialist or American College of Sports Medicine -Exercise Physiologist Certification (ACSM EP-C) (cardiac rehab)
• If the certification you would like to pursue is not listed above, you can request review by department leadership, including justification for this certification and how it impacts the department, done prior to working towards this certification as part of Level 3 achievement. As a general rule, a certification program should involve at least a week of classroom training and an exam at the end.
• 2. Hospital committee member: Regular member (80% attendance or more) of a committee that meets at least once per month or until completion of its assigned task. Actively participates during meetings and does work relevant outside the meetings. Brings information back to the department, disseminates this information, and helps with implementation. Able to demonstrate this to PSM leadership and discuss at goal setting.
• 3. Student clinical instructor for a professional student for an observation clinical OR coordinates and is the main clinical contact for an ROP student or volunteer (orients to duties list, signs off hours, sets schedule).
• 4. Department champion for hospital or department initiative: Regular member (80% attendance or more) that meets at least once a month or until completion of its assigned task. Actively participates during meetings and does relevant work outside of meetings. Brings information back to the department, disseminates this information, and helps with implementation. Able to demonstrate this to PSM leadership and discuss at goal setting. (See Appendix 1 for more detail)
• 5. Assists with a current and regular class or program within PSM department or in conjunction with other departments: May include: active participation in the class (teaching, training); maintenance (assessing and revising content or goals, process improvement, updating to reflect best practices or current state), arranging class/program coverage.
• 6. Assists with formalized implementation of curriculum (approved by leadership) for new/current employees based on subject matter expert status; assists to develop and implement training and competencies; creation of training materials; and ongoing access throughout a new employee's first year of service at Stanford Tri-Valley. Able to demonstrate this to PSM leadership and discuss at goal setting.
• 7. Treats a sub-specialty population (not included in job description). Requires evidence of continuing education competence in the subject area, active caseload within the sub-specialty, and sign off from department leadership for updating of scheduling guidelines. Allows PSM to treat patients that would otherwise be re-directed to a qualified clinic. Examples include:

Preferred Knowledge, Skills and Abilities

• Good physical condition, which requires moderate lifting of equipment and patients.
• Ability to work flexible schedules and varying locations as assigned
• Strong verbal and written English communication skills with the ability to communicate across all levels of the hospital and departments

Licenses and Certifications

• Current American Heart Association Certification for Basic Life Support for Healthcare Providers required.
• Physical Therapy in the State of California required (Codes: OTR, PT, SLP) required.
