Description

• Conduct patient evaluations in both inpatient and outpatient settings to tailor individualized treatment plans
• Provide therapeutic interventions that enhance mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes
• Guide patients and their families through therapeutic goals, discharge planning, and home exercise routines
• Maintain detailed documentation of evaluations, treatments, and patient progress per facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to promote coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (such as neurological or geriatric therapy) are advantageous, but not mandatory

Skills

• Preferred previous experience in the field
• Expertise in treating patients with complex medical and mobility challenges
