ob Overview Aarris Healthcare is seeking compassionate and experienced Per Diem Medical Social Workers (MSWs) to provide home health services throughout the greater Sacramento area including Sacramento, Elk Grove, Roseville, Rocklin, Folsom, Citrus Heights, Carmichael, Rancho Cordova, Davis, Woodland, and surrounding communities. This position is ideal for social workers who are passionate about helping patients and families navigate healthcare challenges while supporting emotional well-being, community resource coordination, and safe care planning in the home setting. Flexible per diem schedules available. Responsibilities Conduct psychosocial assessments for home health patients Evaluate emotional, social, financial, and environmental needs Assist patients and families with coping strategies and adjustment to illness Coordinate community resources and supportive services Provide short-term counseling and emotional support Assist with long-term planning, placement options, and advance care planning Collaborate with nurses, therapists, physicians, and interdisciplinary teams Educate patients and caregivers regarding available resources and benefits Maintain timely and accurate EMR documentation Ensure compliance with Medicare and home health regulations Qualifications Master’s Degree in Social Work (MSW) from an accredited program Current California licensure/certification if applicable Minimum 1 year of healthcare or home health experience preferred Home Health experience strongly preferred Knowledge of Medicare home health regulations preferred Excellent communication and interpersonal skills Ability to work independently in the field Reliable transportation and valid California driver’s license Ideal Experience We welcome candidates with experience in: Home Health Social Work Medical Social Work Case Management Discharge Planning Community Resource Coordination Behavioral Health Support Geriatric Care Caregiver Support Crisis Intervention Psychosocial Assessments Transitional Care Homecare Services Medicare Home Health Interdisciplinary Care Planning #J-18808-Ljbffr Aarriscares
