Clinical Intake Coordinator - LPN

Job Description Hybrid role! Home Health and Hospice. M-F, 9 a.m.-5:30 p.m. Rotating on-call; 2-5 shifts every 4-6 weeks; remote. Holiday rotation. Missouri and Kansas LPN License Required. Responsibilities Facilitate the Home Health & Hospice referral process by receiving referrals from multiple sources. Determine need for additional information from referral sources. Communicate agency acceptance or decline of referrals to varied sources and process accepted referrals. Prioritize and triage hospice referrals. Ensure regulatory compliance during initial screening and intake process. Work on-site 3–4 days per week with option to work remotely 1–2 days per week after orientation. Qualifications Minimum 2 years of applicable experience. Holding a Missouri Department of Health & Senior Services Family Care Safety Registry. Licensed Practical Nurse (KS) – National Council of State Boards of Nursing (NURSYS). Licensed Practical Nurse (MO) – National Council of State Boards of Nursing (NURSYS). Job Details Full Time.
