Resident Care Coordinator LPN

FW1 Full-time Day Shift Tuesday - Saturday This position will help support a 15-resident specialty needs program serving high acuity clients. Firwood Gardens has leadership staff solely dedicated to serving this 15-person program, including 7‑day a week nurse coverage and high staff to resident ratios on all shifts. JOB SUMMARY: A Resident Care Coordinator LPN will work closely with the Health Services team in our Community Based Care facility to ensure the quality care of all residents. This position plays an important part of organizing and assisting with training new care staff and provide ongoing mentorship to our current employees.
