We strive to create a supportive community where every person is valued, heard, and cared for with dignity and respect.The Medical Coordinator Assistant is a delegated clinical support role responsible for assisting with client health support, care coordination, and documentation for clients enrolled in Whole Person Care Clinic programs, including Recuperative Care. This role operates only under the direction and supervision of authorized clinical staff and supports health stabilization, recovery, and continuity of care within scope of practice per program guidelines.The Medical Coordinator Assistant supports CalAIM-covered services in a non-independent capacity and escalates all clinical concerns, changes in condition, or scope-related questions in accordance with WPCC policy and applicable law.Essential Duties and ResponsibilitiesConduct daily non-clinical wellbeing checks and assist with intake processes under authorized supervisionObserve and report changes in client condition to supervising staffProvide basic health education related to hygiene, wellness routines, and medication adherence without clinical interpretationSupport a safe, trauma-informed environment for clients recovering from illness or injurySupport client self-administration of medications in accordance with WPCC procedures and authorized delegationMonitor adherence to medication routines and report concerns promptlyMaintain accurate and timely medication logs and related documentationEscalate medication errors, refusals, adverse reactions, or discrepancies immediatelyAssist with scheduling and coordinating medical, behavioral health, and specialty appointmentsSupport follow-up care activities and referrals under authorized directionParticipate in daily huddles and interdisciplinary team meetings as assignedCollaborate with Case Managers, behavioral health partners, housing staff, and clinical staff to support integrated careDocument health-related interactions, medication support, and coordination activities in the EHR accurately and timelyMaintain documentation in compliance with HIPAA, CMIA, 42 CFR Part 2, OSHA/Cal-OSHA, and WPCC policiesSupport tracking of program metrics and client health outcomes as directedMaintain clean, safe client care areas and support infection-control proceduresAssist with basic facility and supply support related to client care operationsParticipate in required training, competency signoffs, and onboarding activitiesQualifications and RequirementsHigh school diploma or equivalentCurrent EMT CertificationMeet credentialing and certification requirements as defined by WPCC program guidelinesBasic Life Support (BLS) certificationMinimum one (1) year of experience in a clinical, shelter, or social services settingExperience working with individuals experiencing homelessness, chronic illness, or behavioral health challengesFamiliarity with electronic health record systemsCore CompetenciesDelegated clinical support within scope of practice per program guidelinesTrauma-informed, culturally responsive client interactionProfessional communication and teamworkAccurate documentation and digital literacyOrganization, prioritization, and time managementProfessional boundary setting and escalation judgmentConfidentiality and regulatory complianceWorking Conditions & Physical RequirementsOn-site work in recuperative care, shelter, or residential settingsRegular walking between client rooms and care areasAbility to lift to 25 lbs. and assist clients with mobilityExposure to clients with communicable conditions or behavioral health needsUse of PPE and compliance with OSHA/Cal-OSHA and WPCC safety standardsSchedule RequirementsFull-time (40 hours/week), Variable shiftsMust be available for assigned shifts (including nights/weekends)EEO / ADA StatementWPCC is an Equal Opportunity Employer.
