Help Patients Transition Home Safely.PurposeCare is seeking a relationship-driven and operationally strong Hospital Liaison to support hospital partnerships and help patients transition smoothly into home health services.This role is ideal for someone who thrives in a fast-paced healthcare environment, enjoys building strong relationships with hospital teams, and understands the importance of urgency, communication, and patient advocacy during care transitions.As a key member of our team, you'll work directly with hospital case managers, discharge planners, physicians, patients, and families to coordinate referrals, support admissions, and improve continuity of care.What You'll DoCoordinate home health referrals and patient transitions from hospitals into home-based carePartner closely with hospital case managers, discharge planners, social workers, and healthcare teamsManage and follow up on referrals with urgency and strong attention to detailEducate patients and families on home health services and available support optionsConduct regular onsite hospital visits and maintain strong referral relationshipsCollaborate with PurposeCare Intake and Clinical Operations teams to support timely Starts of CareUtilize systems such as: CarePort, WellSky, Axxess, TrellaHelp identify opportunities to improve referral conversion and patient experienceSupport awareness of both home health and private pay service offeringsWhat We're Looking ForRN, LPN, PT, OT, or MSW requiredMinimum of 3 years of experience in: home health, care transitions, clinical liaison work, hospital case management, healthcare operationsStrong relationship-building and customer service skillsAbility to manage multiple priorities in a high-volume healthcare environmentStrong attention to detail and follow-throughExperience working with referral platforms and EMR systems preferredStrong critical thinking and communication skillsComfortable working independently in the field while collaborating closely with operational teamsAs a growing company we provide:A rewarding career making a difference, changing lives & communitiesSchedule FlexibilityOngoing growth, training & professional developmentMentor & referral programsOpportunity to apply for positions at our other PurposeCare locationsOur PurposeCare family is eligible for the following:Medical & Prescription Drug PlanTelehealthDentalVision401KLife InsuranceShort- & Long-Term Disability InsuranceEEOC Statement: All employment decisions at PurposeCare are based on business needs, job requirements, and individual qualifications.
