About the Role: We are seeking a dedicated, board-certified or board-eligible Primary Care Physician or Nurse Practitioner to join our collaborative clinical team. In this role, you will provide comprehensive, high-quality longitudinal care to an established patient panel, emphasizing preventative medicine, chronic disease management, and evidence-based acute care.
Key Responsibilities:
Provide comprehensive outpatient primary care services, including physical exams, routine screenings, and acute/chronic disease management.
Formulate precise diagnostic and treatment plans, ordering and interpreting lab work, diagnostics, and imaging.
Collaborate closely with mid-level providers, clinical staff, and administrative leadership to optimize patient workflows and outcomes.
Maintain accurate, timely documentation within the Electronic Health Record (EHR) system in compliance with regulatory and billing standards.
Participate in quality improvement initiatives and peer review processes.
Qualifications:
MD or DO degree from an accredited medical school.
Successful completion of an accredited Residency in Family Medicine or Internal Medicine.
Board Certification or Board Eligibility (BC/BE) in Family Medicine or Internal Medicine.
Active, unrestricted state medical license and DEA registration.
Strong interpersonal skills with a commitment to patient-centered care and efficient clinical operations.

Qualifications:
Master’s or Doctorate degree from an accredited Nurse Practitioner program (FNP or AGNP preferred).
Active, unrestricted Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) license.
National board certification (AANP or ANCC) as a Family or Adult-Gerontology Nurse Practitioner.
Active DEA license and state prescriptive authority.
Minimum of 1–2 years of outpatient primary care experience is highly preferred (passionate new graduates with extensive clinical rotations may be considered).
