Hospice LPN OpportunityJourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team! This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.Hours: Monday - Friday (3) 12-Hour shifts 7AM-7PM. On-call every other weekendTerritory: Crystal Lake, IL 60014Salary: $26 - $38 hourly. Community health, hospice or medical/surgical experience is preferredCurrently licensed as an LPN/LVN in the practicing stateMust possess current CPR CertificationDemonstrates good verbal and written communication, and organization skillsValid driver's license with an automobile that is insured in accordance with state requirementsUnderstands the philosophy of hospice concept and the needs of the terminally illWhat You'll Do:Provide direct patient care as defined in State Nurse Practice ActImplement current nursing practice following a comprehensive assessment and the plan of careProvide accurate and timely documentation of patient services to reflect the plan of careAssess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of careParticipate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.Provide appropriate pain/symptom management. Evaluates the patient's response to treatments/medications
