Registered Nurse – Home Health & Hospice

Location: Wisconsin – Fond du Lac, Dodge, Columbia & Winnebago Counties
Schedule: Monday–Friday, 8:00 AM–5:00 PM | No weekends or on-call
Visits: 5 per day | Mileage: $0.43/mile

Position Overview

We are seeking a compassionate Registered Nurse (RN) to provide high-quality home-based care and coordinate services for patients throughout Wisconsin.

Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans.
Provide skilled nursing and wound care in patients' homes.
Coordinate care with physicians, social workers, families, facility staff, and interdisciplinary teams.
Manage care transitions and identify appropriate levels of care.
Educate patients and families on treatment plans, disease processes, end-of-life care, and symptom management.
Maintain accurate clinical documentation and advocate for patient needs.
Support case management, pain management, IDG documentation, and recertification as applicable.

Requirements

Current, unrestricted Wisconsin RN License
Wound Care Experience
CPR/BLS certification
Valid driver's license, reliable vehicle, and valid auto insurance
Associate's or Bachelor's Degree in Nursing preferred
Strong clinical judgment, communication, and documentation skills
Case Management certification preferred

Preferred Experience

HCHB (Home Care Home Base) highly desired
MatrixCare or other EMR experience accepted
Hospice, home health, skilled nursing, assisted living, or care coordination experience
Experience with frail/elderly patients and end-of-life care

Training: Candidates without HCHB or hospice experience may be considered. HCHB/hospice training will be provided based on experience.

Compliance Requirements

Skills Competency Checklist, Elder Abuse, HIPAA, Infection Prevention & Control, OSHA training, and documentation of training completed within the past 12 months are required.

Additional Wisconsin locations: Brookfield, Jackson, Waterford, and Sheboygan.
