Job Title Hospital Social Worker Job Description The Social Worker applies principles of Social Work and other behavioral health practices to provide patients and families with assessment, interventions, support, advocacy, and case management as a primary provider and/or as ancillary support to the primary medical provider. In all departments, the Social Worker practices as a member of a multidisciplinary team, providing clinical intervention and support to patients and families. The Social Worker assesses, develops treatment plans, and intervenes to address the needs identified in the assessment and by the team. Utilizes knowledge of culture, age, and developmental stages to develop treatment plans and provide interventions, either as an independent practitioner or as a member of a clinical team. Also may provide support and debriefing to staff following traumatic events. Employee may be required to float to the other facility within Southwest Health Rancho Springs and Inland Valley Hospital based on operational needs, with appropriate orientation and competency validation provided. Responsibilities Provide comprehensive social work services within the acute care setting, including psychosocial assessment, care planning, advocacy, counseling, and discharge planning. Collaborate with interdisciplinary teams to ensure coordinated care. Conduct family and patient education regarding post-discharge resources and community services. Facilitate support groups and congregate services as appropriate. Participate in quality improvement initiatives and hospital-wide meetings to share social work perspectives. Qualifications Acute care experience preferred. Minimum of 2 years as a social worker in any mental health environment required. High School Diploma or equivalent.
