As a member of the Health Support Center (HSC) team, you’ll support staff in our facilities who are interfacing and providing care to patients and community members to positively impact our mission of making communities healthier. Join our team in a 90‑bed Skilled Nursing & Rehabilitation Unit, with 20 rehab beds and 70 LTC beds. Staffed with Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and Medication Aides. The unit cares for patients with CHF, COPD, Diabetes, ESRD, Renal Failure, Dementia, Stroke, and Heart Disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs. How you’ll contribute Accurately performs patient assessments and identifies patient needs Identifies and initiates appropriate nursing interventions Provides care appropriate to condition and age of the patient Performs timely and appropriate documentation relating to medical necessity in the medical record Responsible for completion and revision of the Interdisciplinary Care Plan for each patient Performs timely and accurate QI assessments Why join us Comprehensive Benefits: Multiple levels of medical, dental and vision coverage – medical plans starting at just $10 per pay period, tailored benefit options for part‑time and PRN employees, and more. What we’re looking for A compassionate nurse with a strong foundation in providing holistic and compassionate long‑term care, with a focus on detailed patient assessment, medication administration, and chronic disease management. Applicants should have a current state RN license and possess a degree from an accredited nursing school. Additional requirements include: Basic Life Support certification required within 30 days of hire. One year of experience preferred.
