• Pay Range: 2040 weekly gross

• Schedule: 1pm-9pm

• Available Shift: Days, Monday-Friday, 5 shifts per week
• Start Date: 08/03/2026
• Located In: Jacksonville, FL
• Requirements: Qualified applicants MUST have at least one year of experience in a Medicare-certified home health agency and be comfortable with OASIS (Outcome and Assessment Information Set), have a valid RN license and be willing to obtain FL licensure.
• Contract Length: 13 weeks
• Candidate Type: Local and traveler friendly

Responsibilities:

• Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
• Provide skilled nursing care to patients in their homes, including medication management, wound care, and infusion therapy.
• Perform OASIS assessments and documentation accurately and timely to ensure compliance with regulatory requirements.
• Educate patients and their families on disease management, medication administration, and other health aspects to promote self-care and independence.
• Monitor patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes.
• Provide emotional support and counseling to patients and families coping with chronic illness or end-of-life care.
