The role combines direct patient care with meaningful physician leadership, allowing the successful candidate to influence both individual patient outcomes and the broader quality of care delivered across the centerClinical ResponsibilitiesProvide comprehensive primary care to medically complex and geriatric patientsConduct new participant evaluationsManage acute and chronic conditionsProvide clinical oversight for participants in SNF and ALF settingsParticipate in interdisciplinary care planningLeadership ResponsibilitiesProvide clinical mentorship to physicians and APPsServe as a resource for complex clinical decision-makingSupport provider development and clinical remediationFacilitate clinical education and didactic sessionsSupport site operations and workflow improvementCandidate QualificationsRequired:MD or DOBoard Certified in Family Medicine or Internal Medicine3–5+ years of post-residency clinical experienceExperience caring for geriatric or medically complex populations2–3+ years of clinical leadership, medical directorship, administrative leadership, or GME faculty experienceCalifornia medical licensure or eligibilityPreferred:Geriatrics fellowship or board certificationHospice & Palliative Medicine fellowship or board certificationPACE experienceExperience leading interdisciplinary teamsPopulation health/value-based care experienceExperience mentoring physicians and APPsThe Ideal Candidate:This position is well suited for a physician who wants more leadership impact without giving up patient careYou will have the time and structure to build meaningful relationships with participants, mentor other clinicians, improve clinical processes, and help shape the future of a growing PACE program. xhyhwjd If you are an experienced physician leader passionate about geriatric medicine, interdisciplinary care, clinical excellence, and developing other providers, we encourage you to explore this opportunity.
