LPN Case Manager

The Licensed Vocational Nurse (LPN) Case Manager supports the Transitions of Care program aimed at reducing 30-day hospital readmission rates. Working under the direction of an RN Care Manager, the LPN assists with implementing care plans through appointment scheduling, biometric data monitoring, patient education, home and office visits, and connecting patients with community resources. This role requires clear communication, documentation in the electronic health record, and the ability to work independently within a multidisciplinary care team managing a caseload of approximately 25 30 patients. Reliable transportation is essential as travel to patient homes, hospitals, and provider offices is required. Clinical knowledge of chronic conditions such as CHF and COPD is preferred. This is a non-hands-on nursing role focused on care coordination and case management.
