Clinical Nurse III Post Transplant Coordinator

Department/Unit: Renal Transplant Service Work Shift: Day (United States of America) Salary Range: $65,102.17 - $100,908.37 Responsibilities Clinical Nurse assesses, coordinates, plans, directs, implements and manages the needs of assigned patients throughout the episode of care with a focus on progress toward discharge, including during transitions within the acute care stay. The Registered Professional Nurse is responsible for the delivery and coordination of patient care provided by the caregiving team, ensuring competent, compassionate care is uniformly provided to patients and referring facilities, hospital staff, and the community. Clinical Duties Assists the attending Transplant surgeons, house staff, and Transplant staff with the direct care of hospitalized patients and outpatients on a rotating basis with other clinical care coordinators, including: Facilitation of patient discharges and transplant teaching Attends once a week outpatient Transplant clinics: orders labs, performs injection teaching, and coordinates care Schedules appropriate non-invasive laboratory tests and participates in the interpretation of results and the institution of therapy based on physician guidance Schedules appropriate surgery and tests for patients Discharge planning: reviews discharge instructions, works with the hospital case manager and social worker regarding patient discharge needs Answers telephone inquiries from patients and/or visiting nurses after hospital discharge Provides education to healthcare workers and the public regarding organ transplantation Participates in teaching programs of the Abdominal Organ Transplant Program Additional responsibilities as needed Patient Care & Monitoring Monitors transplant recipients after surgery for complications or rejection and reports findings to the Transplant Physician Reviews lab results and biopsy reports Assesses patient symptoms and clinical status Coordinates follow-up visits and routine post‑transplant evaluations Medication Management Monitors drug levels and adjusts medications based on provider orders Educates patients on medication changes and adherence Provides education and support for lifestyle changes, infection prevention, medication schedules, emotional support, resources, and warning signs of complications in both inpatient and outpatient settings Albany Med Health System is an equal opportunity employer.
