* Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
* Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
* Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
* Monitor and observe residents‘ physical and emotional status; notify the attending physician and responsible party of any change in condition.
* Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
* Complete accurate and timely clinical documentation per facility policy and procedure.
* Coordinate resident admissions, transfers, and discharges according to established procedures.
* Participate in quality assurance activities, staff education, and regulatory surveys as needed.
* Perform other duties as assigned by the Director of Nursing or designee.
What you Need to make a Difference:
* Current, active LPN/LVN license in the state of practice.
* CPR/BLS certification preferred (American Heart Association or American Red Cross).
* Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
* Experience in a supervisory or charge nurse role, preferred.
* Strong clinical observation, medication administration, and documentation skills.
* Knowledge of Federal and State long-term care regulations.
* Effective verbal and written communication skills.
* Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
* Experience in a supervisory or charge nurse role.
* Familiarity with electronic health records (EHR) systems.
* Proficiency with Microsoft Office applications.
