Key Responsibilities\n

  • Conduct thorough psychosocial assessments for patients and their families, recognizing social, emotional, and physical needs upon admission and throughout treatment, to identify and address barriers to care.

  • Collaborate with interdisciplinary healthcare teams to develop and implement comprehensive psychosocial treatment plans tailored to individual patient needs, ensuring effective communication and continuity of care.
  • \n
  • Provide crisis intervention and supportive counseling for patients and families facing illness, assisting them in coping with psychological and environmental challenges related to their conditions.
  • \n
  • Manage an independent caseload, adapting to the varying levels of clinical acuity, while making mental health diagnoses when necessary as part of an integrated treatment plan.
  • \n
  • Engage in proactive case finding, identifying patients and families with potential psychosocial needs and participating in interdisciplinary team rounds to discuss and strategize care plans.

  • Deliver psychoeducation to patients and families regarding the diagnosis, treatment options, and adaptation to new care processes while facilitating transitions from youth to adult services.

  • Document patient interactions meticulously, preparing and maintaining electronic medical records in accordance with organizational policies to support continuity and quality of care.

  • Supervise and mentor MSWs and student interns, providing individual and group clinical supervision to nurture the next generation of mental health professionals.

  • Participate in quality assurance initiatives to enhance service delivery frameworks and contribute to policy development pertaining to social work services.

Qualifications\n

  • A Master's Degree in Social Work, Marriage and Family Therapy, Professional Clinical Counseling, or a related behavioral science field from an accredited university.

  • Current licensure in California as either a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).

  • Demonstrated ability to apply effective counseling techniques and social work theories.

  • Strong written communication skills with proven ability to create comprehensive case narratives.

Compensation and Benefits\n

This position offers a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience.
