Physical Therapist Assistant - Home Health

Overview The Physical Therapist Assistant works under the direction of a licensed Physical Therapist to perform procedures and treatment activities based on the established Physical Therapy plan of care. This role reviews physician referrals, patient reports, Physical Therapy evaluations, treatment goals, and the medical record to support safe and effective patient care. Responsibilities Provide treatment, education, documentation, and patient support across assigned clinical settings under the supervision of a Physical Therapist. Communicate patient progress or changes in condition to the Physical Therapist and support the care team by helping maintain quality, safety, and regulatory standards. Qualifications Ability to perform assigned procedures and treatment activities under the direction of a licensed Physical Therapist, per established plan of care.
