Benefits 401(k) 401(k) matching Dental insurance Free food & snacks Health insurance Paid time off Vision insurance Driver Files and Eligibility Driver files: For new drivers: Make sure driver completes all necessary paperwork Follow FMCSA guidelines to contact previous employer regarding driving record and testing results Copy of driver record from state agency Send driver for initial drug screen Review DMP rules, code of conduct and drug policy – have them sign acknowledgement Obtain copy of medical card and driver’s license Prior to first drive, send for alcohol screen DMP driver agreements and substance abuse policy For current drivers: Annual declaration of violations (to be completed by driver) Obtain a copy of driving record from state agency (every year) Send drivers for randomly selected drug and alcohol screens For terminated drivers: Document files with termination paperwork Remove them from testing pool Other eligibility requirements: Maintain a calendar of expiration dates for current driver medical cards and licenses Maintain records of random drug and alcohol testing, as well as results Insurance: Communicate with Mary regarding drivers to be added or removed from DMP insurance (secure appropriate info from drivers if needed) Notify of any accidents that may need to be reported Driver Training Maintain records of driver training (dates and attendance) Conduct mock audits (when needed) #J-18808-Ljbffr David-Michael-Productions-Inc
