We are the trusted resource for HIV and STI services, fostering a culture of love, service and hope in every interaction.” Overview: Provides medical case management services under the Kentucky HIV/AIDS Care Coordination Program (KHCCP), including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs.

KEY RESPONSIBILITIES

Assist Housing Coordinator with Housing programs. Promote a Housing First approach with clients. Follow HOME TBRA and HOPWA guidance given by Kentucky Housing Corporation and HUD. Build relationships with property managers and leasing agencies

QUALIFICATIONS

A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred. Candidates with case management experience of two years or more will also be considered as viable applicants. Experience working in a nonprofit organization or a strong desire to work in service‑driven environment. 100% Commitment to Matthew 25's mission, vision and values Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
