Behavioral Health Care Coordinator (Onsite - Fairfield, CA) About the Role We're looking for a compassionate and detail‑oriented Behavioral Health Care Coordinator to join our growing health plan team. In this role, you'll support members with behavioral health needs by helping them access the right care and resources to improve their overall health outcomes. You'll work closely with providers, case managers, and community organizations to ensure members receive coordinated, high‑quality care in a fast‑paced and collaborative environment. What You'll Do Manage a specialized caseload of members with behavioral health needs Serve as a key point of contact for members calling in for behavioral health services Conduct screenings using established tools and help connect members to appropriate providers Collaborate with physicians, psychiatrists, counselors, and care teams to coordinate services Support members transitioning from higher levels of care to community‑based settings Maintain accurate documentation in the case management system Act as a resource for internal teams on behavioral health‑related cases Help connect members with county‑based services and community organizations What We're Looking For Strong communication skills (written and verbal) Excellent organization, attention to detail, and critical thinking skills Ability to multitask and work independently in a fast‑paced environment Experience working with healthcare systems, managed care, or behavioral health services Comfortable handling inbound calls and documenting member information Ability to collaborate across teams and systems of care Work Environment & Schedule Location: Fully onsite in Fairfield, CA Schedule: Monday – Friday, 8:00 AM – 5:00 PM Job Type & Location This is a Contract position based out of Fairfield, CA.
