Job Summary

HireCare Services is partnering with a leading skilled nursing facility in Warwick, RI to recruit a compassionate and dependable Licensed Practical Nurse (LPN) .

As an LPN, you will provide high-quality nursing care to residents while working alongside a dedicated nursing team in a supportive, resident-centered environment. Responsibilities

• Provide direct nursing care to residents in accordance with facility policies, physician orders, care plans, and safe nursing practices

• Assist RNs and other nursing staff with daily duties and resident care

• Administer medications and treatments as ordered

• Monitor residents for changes in condition, side effects, or adverse reactions and report concerns appropriately

• Perform routine nursing tasks, including vital sign checks and wound care

• Document resident care and progress accurately and in accordance with facility policies and regulations

• Collaborate with the interdisciplinary care team to develop and implement individualized care plans

• Participate in care conferences and care plan updates

• Promote residents' comfort, dignity, privacy, and independence

• Communicate effectively with residents, families, and members of the healthcare team

• Follow all applicable healthcare regulations, safety procedures, and facility policies

• Promote teamwork, professionalism, and effective communication

Qualifications

• Graduate of an approved Licensed Practical Nursing (LPN) program

• Current, unencumbered Rhode Island LPN license

• Current CPR certification

• Previous long-term care or skilled nursing experience preferred, but not required

• Strong clinical judgment, communication, and teamwork skills

• Ability to handle multiple priorities with professionalism and tact

• Reliable, compassionate, and team-oriented
