Responsibilities:

• Provide direct care and assistance to individuals in their homes or healthcare facilities
• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
• Monitor and record vital signs, such as blood pressure and temperature
• Lift and transfer individuals who may have limited mobility or require assistance
• Provide hospice care and support to individuals and their families during end-of-life stages
• Maintain confidentiality and adhere to HIPAA regulations when handling sensitive information
• Offer direct support to individuals with disabilities or special needs
• Assist with medication reminders and administration, as directed by healthcare professionals
• Clean and maintain a safe environment for individuals under your care

Skills:

• Experience with ADLs and providing personal care to individuals
• Proficient in taking vital signs accurately and documenting them appropriately
• Ability to lift heavy objects or individuals, as required for the role
• Familiarity with hospice care practices and providing emotional support to patients and families
• Knowledge of HIPAA regulations and maintaining privacy and confidentiality
• Experience in direct support roles, such as working in assisted living facilities or nursing homes
• Valid driver's license for transportation purposes, if required for the position
• Strong organizational skills and attention to detail when cleaning and maintaining a clean environment
