In this role, you will:

• Conduct evaluations of patients in both inpatient and outpatient environments, while designing tailored treatment plans that prioritize individual needs.
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality.
• Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes.
• Provide education to patients and their families regarding therapeutic objectives, discharge processes, and home-based exercises.
• Maintain thorough documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards.
• Engage actively in care team rounds and interdisciplinary meetings to promote seamless coordination in patient care.

To thrive in this position, candidates must hold:

• A Doctorate in Physical Therapy (DPT) or an equivalent accredited degree.
• A valid state license as a Physical Therapist.
• CPR certification is preferred, along with specialized certifications (e.g., in geriatrics or neurology) if available, although not mandatory.
