Registered Dietitian

BAYVIEW HUNTERS POINT MULTIPURPOSE

POSITION PURPOSE The Registered Dietitian is responsible for providing professional nutrition services to participants enrolled in the Adult Day Health Care (ADHC) program. This position ensures compliance with all applicable state regulations and professional standards while supporting participants' overall health, wellness, and quality of life. The Registered Dietitian conducts nutritional assessments, develops individualized care plans, provides nutrition education and counseling, and collaborates with the interdisciplinary team to promote optimal health outcomes and safe nutritional practices DUTIES AND RESPONSIBILITIES Develop, implement, and oversee the nutrition services program in accordance with Adult Day Health Care regulations and organizational policies Conduct comprehensive initial nutritional assessments and ongoing reassessments for participants at least every ninety (90) days or as otherwise required Develop, implement, and update individualized nutrition care plans based on participant needs and goals Participate in interdisciplinary team meetings and collaborate with healthcare professionals to coordinate participant care Provide nutrition counseling and education to participants and families to promote healthy eating habits and disease prevention Monitor participant nutritional status and recommend appropriate interventions as needed Maintain accurate, timely, and compliant documentation of assessments, services, care plans, and participant progress Complete required reassessments following significant changes in participant condition, hospitalization, or other life events Provide oversight of meal service operations to ensure compliance with nutritional standards, food safety requirements, and sanitation procedures Monitor kitchen practices to ensure proper use of measuring utensils, personal protective equipment, and safe food handling procedures Ensure compliance with Title 22 California Code of Regulations and other applicable state and federal requirements Support quality improvement initiatives and program evaluations related to nutrition services Perform additional duties as assigned to support program operations and participant care MINIMUM REQUIREMENTS Registered Dietitian (RD) credential in good standing through the Commission on Dietetic Registration (CDR) required Proof of current registration required Minimum of one (1) year of experience providing clinical nutrition services preferred Experience working with older adults, individuals with disabilities, or within a healthcare setting preferred KNOWLEDGE, SKILLS, AND ABILITIES Strong knowledge of nutrition science, medical nutrition therapy, and disease management Knowledge of state and federal regulations related to nutrition services and Adult Day Health Care programs Ability to conduct nutritional assessments and develop individualized care plans Strong verbal and written communication skills Ability to effectively educate participants, families, and caregivers regarding nutrition-related topics Excellent organizational, documentation, and time management skills Ability to work independently while maintaining collaboration with interdisciplinary team members Strong attention to detail and commitment to regulatory compliance Ability to maintain confidentiality and exercise sound professional judgment Commitment to ethical practice and participant-centered care #J-18808-Ljbffr
