Job Summary

As a Certified Nursing Assistant (CNA) , you will provide compassionate, one-on-one care and support to clients in their homes while following an individualized care plan.

You will play an important role in helping clients maintain their comfort, dignity, independence, safety, and quality of life .

The ideal candidate is compassionate, dependable, detail-oriented, organized, and committed to providing personalized and attentive care. Building genuine relationships with clients and providing companionship are important parts of this position.

Strong verbal and written communication skills are required. Previous Home Health, Hospice, private-duty, or senior care experience is a plus.

Responsibilities

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility.
• Assist clients with safe transfers and movement in and out of beds, baths, wheelchairs, and automobiles, as appropriate.
• Provide assistance with ambulation and mobility devices while following the client's care plan and safety precautions.
• Provide personal care while maintaining the client's dignity, privacy, independence, and comfort.
• Monitor and observe clients for changes in condition, behavior, mobility, or overall well-being.
• Report changes in the client's condition or other concerns promptly to the appropriate supervisor or care team member.
• Assist with meal preparation and routine household activities as needed.
• Change bed linens, perform laundry, and help maintain a clean, safe, and comfortable home environment.
• Provide companionship through conversation, reading, games, and other appropriate activities.
• Encourage clients to participate in exercises, hobbies, and activities as appropriate to their care plan.
• Maintain accurate and timely records of client care, condition, progress, and concerns.
• Follow the client's individualized care plan and all company policies and procedures.
• Maintain professional and respectful communication with clients, families, supervisors, and members of the care team.
• Protect client privacy and maintain confidentiality at all times.
• Promote client safety and help identify and reduce potential fall and other safety risks within the home.

Qualifications & Requirements

• Graduate of an approved Certified Nursing Assistant (CNA) program .
• Current and valid CNA certification required.
• Minimum of 3 years of professional caregiving or CNA experience preferred .
• Previous Home Health, Hospice, senior care, hospital, facility, or private-duty experience is a plus.
• Valid Driver's License required .
• Current CPR Certification required.
• Reliable transportation and the ability to travel to client locations.
• Strong verbal and written communication skills.
• Excellent interpersonal and relationship-building skills.
• Ability to work independently while following an individualized care plan.
• Ability to provide compassionate, respectful, and client-centered care.
• Ability to maintain professional boundaries and client confidentiality.
• Strong organizational and time-management skills.
• Ability to safely assist clients with mobility, transfers, and activities of daily living.
