Case Manager

Overview: As a Case Manager at Asurgent Health Addiction Treatment Center, you will play a crucial role in providing comprehensive support and assistance to individuals undergoing addiction treatment. You will work closely with clients, their families, and a multidisciplinary team to develop and implement personalized treatment plans aimed at promoting recovery and holistic wellness. Responsibilities: Assessment and Evaluation: Conduct thorough assessments of clients' needs, strengths, and challenges related to addiction treatment. Collaborate with clients and other team members to develop individualized treatment plans based on assessment findings. Treatment Planning and Coordination: Help develop comprehensive treatment plans that incorporate evidence-based interventions and address clients' physical, psychological, and social needs. Coordinate with healthcare professionals, counselors, and other service providers to ensure seamless delivery of services and continuity of care. Monitoring and Support: Provide ongoing support and guidance to clients throughout the treatment process. Monitor clients' progress, evaluate the effectiveness of interventions, and make adjustments to treatment plans as needed. Advocacy and Resource Referral: Serve as an advocate for clients, ensuring their needs are met and their rights are respected. Identify and connect clients with community resources, support groups, and other services to facilitate their recovery journey. Crisis Intervention: Respond promptly and effectively to crises or emergencies involving clients. Collaborate with team members to develop crisis management plans and ensure client safety. Documentation and Reporting: Maintain accurate and up-to-date documentation of client assessments, treatment plans, progress notes, and other relevant information. Generate reports as required by regulatory agencies and funding sources. Qualifications: Previous experience in addiction treatment or mental health services strongly preferred. Strong communication, interpersonal, and problem-solving skills. Ability to work effectively both independently and as part of a multidisciplinary team. Knowledge of relevant laws, regulations, and ethical standards related to addiction treatment and case management.
