Responsibilities

• Provide licensed practical nursing care to adult residents under the supervision of an RN
• Administer prescribed medications and treatments in line with orders and facility policy
• Monitor vital signs, perform focused assessments, and document observations in the EMR
• Support activities of daily living and resident mobility, including memory care needs and orthopedics-related care
• Observe residents for changes in condition and communicate promptly to the supervising RN
• Float to all units within the building as needed, while maintaining safety and infection control standards

Requirements

• Current New York Licensed Practical Nurse license (active)
• BLS certification
• Minimum 1 year of LPN experience in a long-term care or skilled nursing setting
• Graduate of an approved practical nursing program
• Ability to work in a skilled nursing facility and float to multiple units within the same building
• Experience with PointClickCare EMR preferred; on-site training provided if needed

• Graduate of an approved practical nursing program
• Licensure: Licensed Practical Nurse, state of New York
