Care Coordinator: Patient Advocate & Wellness Navigator

Foundations for Living

Foundations for Living is seeking a Care Coordinator to engage and support members in overall health and wellness. This role entails coordinating services, advocating for members and increasing health knowledge. The ideal candidate will have a High School Diploma, with a preference for a degree in Community Health. The position is hybrid, primarily in McAllen or Laredo, Texas, and requires a minimum of 2 years’ experience in healthcare.
