Provide compassionate care to clients in the comfort of their own homes throughout Rhode Island.

The Home Care Skilled RN Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The Home Care Skilled RN Case Manager conducts skilled nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The Home Care Skilled RN Case Manager for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our Home Care Skilled RN Case Manager is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.

Qualifications for a Home Care Skilled RN Case Manager:

• Current unencumbered RN license in Rhode Island.
• Minimum of one year nursing experience, home care experience preferred.
• CPR certification.
• Reliable transportation, current clean driver's license, automobile insurance.
