RN will do 5 to 6 patient visits per day (Monday thru Friday 8:30 am to 5 pm). Territory: Brooklyn NY Implements and evaluates the home care needs of patients, including the assessment of a safe living environment, proper resources and support system of caregivers. Communicates a plan of care for the patient between the physician, physical therapist, social worker and home health aide services. Reconciles patient's medications with the Physician. Assess patient and caregivers' knowledge of medications to instruct on medication use, actions, side effects, dosage, routes, and frequencies. Evaluates and implements appropriate interventions for patient's wounds, signs and symptoms of disease processes and any complications. Educates patients and caregivers on treatment of disease and tools for disease management. Proficient in placing wound vacs and Unna Boots when instructed by the physician. Completes and submits OASIS assessments, reassessments, transfers, resumption of care, discharges, and significant changes in condition in accordance with Agency defined time frames. Reviews patient insurance information to coordinate activities involved in each individual care plan. Assures that pertinent findings are shared with family and professional caregivers. Enters all relevant patient visit documentation into the KanTime EMR Qualifications RN with active New York State Nursing license in good standing Homecare / CHHA experience preferred OASIS experience highly preferred RN Must have a car / vehicle to be considered Jobot is an Equal Opportunity Employer.
