Job Description
Job Title: Licensed Practical Nurse (LPN)

Must-Have: Prior experience with PCC; experience measuring wounds.

Description:


MUST INCLUDE DL IN SUBMITTAL.

No duo shifts are allowed until extension.

24 hours to accept offer.

Health items need to be collected within 48 hours.

Need to be cleared with the exception of prints by 4 PM EST the Friday before start.

Must use facility time clock.

If badge is lost, replacement fee is required.

Forget badge first time will result in a verbal warning; second time will result in a written warning; third time will result in a second written warning; fourth time will lead to termination.

Facility has a total of 300 beds divided into six units.

Accepts long term care, skilled care, rehab, and dementia residents aged 18 and older.

Hospice care is accepted in-house with two dedicated hospice rooms.

Floor nursing roles include medication and treatment administration, blood glucose monitoring, insulin administration, IV medications, TPN, tube feedings, wound care, lab draws, Foley catheter care, tracheostomy care, ADL assistance, assessment charting, incident reporting, and monitoring vitals.

Registered Nurses only will perform central line cares, chest tube care/management, IV starts, admission orders, and fall assessments.

Majority of floor nurses are LPNs.

Three Nurse Managers are assigned to two units Monday through Friday.

On weekends, one Nurse Manager oversees all units.

There will be two additional nurses on each unit (one Charge Nurse and one Medication/Treatment Nurse), staffing dependent.

Evenings will have an RN Supervisor and two additional nurses on each unit.

Nights will have RN Supervisor as staffing allows with one floor nurse (RN or LPN) per unit.

Unit staffing may include seven total nurses across six units.

Nursing duties will consist of both RNs and LPNs.

House MD and NP are available by phone after hours.

Interdisciplinary team includes Physical Therapy, Occupational Therapy, Speech Language Pathology, Case Managers, Social Workers, and Dieticians.

Respiratory Therapy is not on-site 24/7.

Pharmacy is on-site Monday through Friday during normal business hours.

Medication is administered from pill bottles.

Emergency drug closet access is provided to RN Supervisor outside of pharmacy hours.

Shifts are 8.25 hours long, including a 30-minute unpaid lunch break.

Weekend requirements include every other weekend, with the option to work more if desired.

Holiday requirements exist, but there is no set rotation.

Schedule is prepared two weeks in advance.

No call or standby requirements exist; employees will work their scheduled shifts.

Efforts are made to keep nurses in consistent areas, but may be assigned to any area as needed.

Scrub color is any clean, professional scrub except camouflage.

Three-day specific orientation is required for this role.
