*Contingent Clinical Care LVN\/LPN - MC

Job Summary LPN/LVN Position:

- Position: Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) Temporary/Contract
- Work Setting: Primary Care, Urgent Care, or Emergency Room (not long-term care)
- Schedule: Full-time, 40 hours/week including every other weekend and some holidays; 7 10 hour shifts; 3-week required training/orientation; 6-week schedule blocks

Key Responsibilities:
- Provide high-quality, evidence-based nursing care to patients
- Assess patient needs and perform routine and complex patient evaluations
- Administer medications (oral, injectable, IV, and topical)
- Monitor and document vital signs, identify abnormalities, and record responses to treatment
- Conduct medication reviews and screen for potential side effects/interactions
- Develop and administer mental health assessments (depression, mood, cognition)
- Maintain accurate and up-to-date medical records using EMR (Epic preferred)
- Coordinate patient access to resources for medication, self-care, and disease management
- Communicate effectively with patients, families, and healthcare teams
- Maintain a clean, organized, and well-stocked clinic environment

Required Skills & Experience:
- Minimum 1 year of experience in Primary Care, Urgent Care, or Emergency Room
- Phlebotomy, vaccine administration, and point-of-care testing experience
- Proficiency with EMR systems (Epic preferred)
- Strong clinical knowledge, critical thinking, and communication skills

---

Licensure & Certification:
- Active, unrestricted LPN/LVN license in good standing (no disciplinary actions, compact license for primary state of residence)
- Current BLS certification (valid at least 30 days past start date)

Additional Requirements:
- Willingness to work variable shifts, weekends, and holidays as scheduled
- Ability to complete 3 weeks of initial training without interruption
- Must provide license and BLS documentation upon submission
- Understands temporary/contract nature of the role (conversion to permanent possible but not guaranteed)
- Must confirm distance from home to clinic and availability for required schedule
