Your Role and Responsibilities

• Performing thorough patient assessments, ordering necessary diagnostic tests, and developing personalized care strategies
• Diagnosing and treating acute, episodic, or chronic illnesses collaboratively with physicians, or independently if credentialed
• Providing education to patients and their families on prevention, wellness, and disease management
• Prescribing medications and therapies within your scope of practice
• Involving in patient follow-ups and chronic care management
• Working collaboratively with interdisciplinary teams to deliver comprehensive, holistic care
• Maintaining accurate and comprehensive documentation within electronic health records (EHR)

Educational Background

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Graduate of an accredited Nurse Practitioner program

Licensure and Certification Requirements

• Current Nurse Practitioner licensure in Illinois
• National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or an equivalent specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to obtain within 90 days)
