Now Hiring: Full-Time Physical Therapist Floater Home Care & Outpatient | $48-$52/Hour | Monday-Friday | 40 Hours/Week | No Weekends Community Physical Therapy (CPT) is seeking a Full-Time Physical Therapist Floater to join our Continuum of Care (COC) team, providing home care and outpatient therapy services across Alden Assisted Living and Independent Living communities.

What You'll Do

• Provide physical therapy services across Alden Assisted Living and Independent Living communities.
• Deliver individualized, patient-centered care that helps patients achieve their goals.
• Build meaningful relationships with patients while supporting continuity of care.
• Collaborate with an interdisciplinary therapy team to promote the best possible outcomes.
• Complete documentation for home care in Homecare Homebase (HCHB) and outpatient services in TheraOffice.

What We're Looking For

• Current Illinois Physical Therapist license.
• Strong clinical, communication, and organizational skills.
• Ability to work independently while collaborating with a supportive team.
• A passion for helping patients maintain independence and improve their quality of life.
