Description

American Traveler is seeking an experienced RN for a Case Management position in an adult inpatient setting, requiring active Case Management experience and proficiency with Epic EMR.

Details

• Day shift, 8-hour shifts (8:30 AM – 5:00 PM)
• Five 8-hour shifts per week
• Adult Medicine/Surgery inpatient Case Management unit
• Caseload focuses on adult patients requiring care coordination from triage through safe transition to home or post-acute settings
• No weekends, no holidays, and no on-call requirements
• EMR: Epic (experience required)
• Post-acute facility referral platform experience required (e.g., NaviHealth or AllScripts)

Requirements

• Active MD or Compact RN license currently in hand
• Minimum 2 years of RN experience required
• Current inpatient Case Management experience required
• Current BLS certification required
• Experience at a comparable facility size (500+ bed count) required
• Proficiency with Epic EMR required
• Experience with post-acute referral platforms such as NaviHealth or AllScripts required

Additional Information

• Coordinates care for a select caseload of adult patients from triage through discharge and transition to home or post-acute settings
• Proactively communicates with payors to obtain approval for treatment plans and required resources
• Collaborates with multidisciplinary teams, patients, and families to facilitate effective transition planning
• Arranges transitions to post-acute settings including nursing facilities, acute inpatient rehab, and community resources such as local shelters
• Maintains and enters patient information in all departmental systems per operational guidelines
